• Collaborate with an inter-disciplinary care team to improve member outcomes through culturally competent care
• Conduct strength-based assessments and develop individualized care plans in partnership with members
• Facilitate communication and coordination of care among team members and community resources
• Monitor member satisfaction and adjust care plans as necessary to meet evolving needs
• Document all care coordination activities and ensure compliance with relevant standards and regulations

• Nursing degree from an accredited program or Bachelor’s degree in a healthcare field, or equivalent experience
• Licensure as a Registered Nurse, Professional Clinical Counselor, or Social Worker
• Minimum of three (3) years of experience in nursing, social work, or a related healthcare profession
• Knowledge of local, state, and federal healthcare laws and regulations
• Current unrestricted clinical license in the state of practice

• Advanced degree associated with clinical licensure
• Three (3) years of experience in Medicaid and/or Medicare managed care
• Case Management Certification
• Strong understanding of quality metrics and disease management practices
• Experience in care coordination and case management
