- Perinatal Care has an exciting opportunity for a Full-Time Case Worker 2 (Housing and Employment Specialist) to work in Miami.
The Case Worker 2 (Housing and Employment Specialist) monitors outcomes, data collection, and record keeping, and coordinates services for the respective population, including providing information about professional development opportunities, quality improvement strategies, and counseling to improve the client’s social and psychological conditions. The Case Worker 2 (Housing and Employment Specialist) assists those experiencing difficult situations by helping them find the community resources/services that they need contributes to the development of family support plans and monitors its implementation and modification.
Core Responsibilities:
* Conducts situational analysis, which may be at home and/or office visits, mentors, and provides technical assistance as needed.
* Collaborates with the service agencies and program leadership to provide and administer services appropriately.
* Develops and provides training to family support networks and/or care providers on various topics related to quality early care and education.
* Maintains and reviews records and files to ensure compliance with project guidelines.
* Collaborates with all funding sources to ensure the availability of adequate resources.
* Maintains current knowledge of services available and changes applicable rules and regulations.
* Attends meetings and training, which may include the Early Learning Coalition, United Way, and the Quality Counts Career Center to enhance developmental potential.
* Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions:
This direct service position is responsible for providing maternal child health services to clients (pregnant women and infants) participating in the Healthy Start/CIR CONNECT Programs and related special projects. Duties also include quality assurance and quality improvement activities related to service delivery. Must be willing to conduct home visits with personal vehicle to provide services to clients and interact with other community providers.
Position is salaried and requires working evenings and weekends as needed to ensure that job duties are met.
* Provide maternal child health services that may include: making initial contacts/phone calls, recruitment, and referral of eligible participants; conducting client assessments; developing individualized care plans to address service needs and risk factors; and providing comprehensive care coordination and wraparound services including parenting education, breastfeeding support, tobacco cessation, inter-conception care education, childbirth education, and psychosocial referral services as needed.
* Assess client needs for housing and vocational training and make referrals as needed; establish partnerships with local housing providers and employers to further facilitate referrals.
* For high-risk clients with substance use or substance exposure risk-factors, develop a plan of care to address risks, assess motivation to engage in treatment, and make treatment referrals.
* Work in conjunction with supervisor and multidisciplinary service team to ensure implementation of coordinated services as related to program participant needs.
* Provide case management/care coordination referrals to participants when appropriate for the range of maternal child health psychosocial and behavioral health needs.
