Bethe Best Hospice RN You Can Be
If you meet these qualifications, we would love to meet you:
- Conduct comprehensive nursing assessments and develop individualized care plans in coordination with the interdisciplinary team, addressing physical, emotional, social, and spiritual needs.
- Maintain timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements.
- Coordinate care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of care.
- Provide education and support to patients and caregivers, and respond to urgent needs during on‑call or admissions assignments.
- Supervise LPNs, LVNs, and hospice aides as needed, and participate in IDG meetings, patient conferences, and ongoing clinical education.
- Support pre‑hospice services by evaluating patient status via phone, telehealth, or in‑person visits and identifying needs for additional care or services.
