Description

The position functions as a liaison between assigned patients and the clinical care team, as well as supporting departments, to assure patients’ expectations are completely satisfied. Considers and incorporates special needs, such as logistics and scheduling of non‑traditional patients. Builds relationships with patients and physicians, to assist in the communication and implementation of a plan of care including appointments, transportation, education, provision and/or enablement of support services and representation within the multidisciplinary care environment.

Uses independent judgment, critical thinking, problem‑solving and advanced communication skills to achieve the highest level of patient satisfaction in accordance with Baptist Health service standards and in compliance with regulatory guidelines. Must work in collaboration with Nurse Navigator and clinical care staff on resolution of critical aspects of care that require clinical judgement and safe delivery of patient care. Estimated pay range for this position is $20.62 – $24.95 / hour depending on experience.

  • Associates.

Licenses &

Certifications:

  • NCBC Breast Patient Navigator Certification – Advocate.
  • TBICC Certified Patient Experience Professional.
  • AONN Oncology Patient Navigator – Certified Generalist.

Additional Qualifications:

  • Associates degree AA or AS required or enrolled in Bachelors degree program to be completed within 2 years of job entry.
  • For Oncology:
    Oncology Patient Navigator – Certified Generalist (OPN‑CG) required within 2 years of job entry date.
  • For Women’s Institute:
    Certified Navigator‑Breast Advocate through the NCBC (National Consortium of Breast Cancers) required within 2 years of job entry date.
  • For Marcus Neuro Institute:
    Certified Patient Experience Professional (CPXP) required within 2 years of job entry date.
  • Bachelor degree desired and 5 years relevant work experience in customer service.
  • Healthcare experience required with 1 year in navigation/care coordination.
  • Experience in the management of disease process across the span of life, ranging from infancy to geriatric care.
  • Individuals with experience in managing chronic illness in the following disease sites will be considered:
    Neurology, Dermatology, Breast, Cardio‑Thoracic, Genitourinary, Gastric, Gynecology, Head and Neck, Endocrinology, Hematology to include solid and liquid tumors.
  • Proficient English communication skills required, written and verbal.
  • Bilingual English/Spanish preferred.
  • Proficient computer skills (i.e. Microsoft Office Word, PowerPoint, Excel, etc.).
