Position: Clinical Coordinator- Emergency Services (Full Time, Days)

Overview

Clinical Coordinator
- Emergency Services (Full Time, Days) – 300839. Provides clinical leadership and compassionate, family-centered care for emergency and trauma patients. Applies evidence-based practice, problem solving, and a multidisciplinary approach in the plan of care. Performs rapid assessment, treatment, patient and family education, and ensures continuity of care through discharge planning. Works under the supervision of the Nursing Manager/Director.

Qualifications include a minimum of 2 years RN experience, CPR/CPR-PI (CPI) within 1 year in this position, experience as a Charge Nurse, PALS within two years as specified by the department, and Florida RN licensure.

Essential duties and responsibilities, education, and credentials are aligned to support department and hospital goals and patient safety initiatives.

Essential Duties and Responsibilities
  • Coordinate and continuously monitor patient flow and throughput, using appropriate resources from admission, transfer, or discharge, and facilitate processes in a timely manner.
  • Identify clinical strengths and opportunities, re-assign patient assignments to support clinical competency, and coach/mentor employees to improve performance and outcomes.
  • Adjust staffing to census fluctuations, collaborating with Operations Administrators and Nursing Staff Office to meet hospital needs.
  • Promote cost containment and maintain fiscal performance by ensuring appropriate supplies and par levels.
  • Support departmental goals and the hospital strategic plan; actively participate in nursing governance councils and unit-based committees.
  • Conduct department huddles, communicate hospital and department updates, and share HCAHPS patient perception data.
  • Foster staff empowerment and open communication; lead and guide staff to build a high-performance, engaged team.
  • Collaborate with all levels of the hospital interdisciplinary team to promote a team-based approach department- and hospital-wide.
  • Interact with patients and families to review plan of care, solicit input, and communicate concerns to physicians.
  • Ensure education about disease processes and treatments is documented in the electronic health record for patients and families.
  • Administer direct patient care as needed, especially during high census periods or when other resources are limited.
  • Oversee and assist staff in the delivery of patient care through the nursing process of assessment, planning, intervention, and evaluation.
  • Assume patient care coordination on designated shifts, including accountability for patient throughput and outcomes.
  • Identify opportunities to improve processes and participate in projects to enhance workflow using Lean methodologies and tools.
  • Assist and support RNs and the team with code blue and rapid response efforts.
  • Collaborate and round with a multidisciplinary team in daily care planning rounds and foster a Family Centered Care environment.
  • Communicate patient progress, including critical findings and changes in condition.
  • Implement and document the plan of care in collaboration with other disciplines and services to ensure safe, efficient care.
  • Participate in the performance review process through chart reviews and validation of timeliness and completeness of documentation.
  • Ensure staff adherence to Joint Commission, CMS, DOH, and other accrediting agency regulatory requirements.
  • Ensure staff adherence to hospital policies and procedures.
  • Promote an error-free working environment by delivering high-quality, safe care per hospital policies; conduct prevention rounding and promptly follow up on concerns.
  • Provide continuous supervision and observation of behavioral health patients and adhere to non-violent crisis intervention strategies (CPI).
Knowledge/Skills/Abilities
  • Career development:
    National certification preferred within 5 years of hire.
  • BSN degree or current enrollment in a BSN program.
  • Proficiency with Microsoft Office applications (including Excel and Word).
  • Trauma certification preferred.
  • Pediatric nursing in an acute care setting preferred.
  • Effective verbal and written communication skills.
  • Ability to positively influence, hold self and others accountable, and collaborate in a multidisciplinary team.
  • Ability to lead a team to achieve business results.
