The Department of Family Medicine and Community Health is seeking a full-time Case Worker to work on our Miami campus. The Case Worker coordinates services for the target population, provides information about professional development opportunities, quality improvement strategies, and counseling to improve clients’ social and psychological conditions. The Case Worker assists individuals experiencing difficult situations by helping them access community resources and services.
CoreJob Functions
- Conducts situational analysis via home and/or office visits; mentors and provides technical assistance as needed.
- Collaborates with service agencies and program leadership to provide and administer services appropriately.
- Develops and provides training to family support networks and/or care providers on topics related to quality early care and education.
- Maintains and reviews records to ensure compliance with project guidelines.
- Collaborates with funding sources to ensure availability of adequate resources.
- Maintains current knowledge of services and changes to applicable rules and regulations.
- Attends meetings and trainings (e.g., Early Learning Coalition, United Way, Quality Counts Career Center) to enhance developmental potential.
- Adheres to University and unit-level policies and safeguards University assets.

Core Qualifications- Bachelor’s degree in a relevant field
- Minimum 1 year of relevant experience
- Bilingual preferred

General Description (Purpose and Function):
The Case Worker will assist patients with tobacco cessation efforts, educate health care providers on best practices for tobacco cessation, and support the use of appropriate pharmacotherapy products. The Case Worker will plan, conduct, and evaluate support groups for patients trying to quit smoking, in coordination with health care providers. The Case Worker will provide tobacco prevention education and information, and support community efforts to create/sustain tobacco-free environments and worksite wellness programs in partnerships with the organizations.
Duties and Responsibilities
Systems Change Activities (50%)
- Work with Systems Change Consultant to spearhead a tobacco program system-change initiative targeting hospitality industry employees within Miami-Dade County; identify key contacts and champions for change.
- Conduct needs assessments for tobacco system change at selected hospitality settings and draft reports to initiate discussions with facility administration; formalize relationships via MOA.
- Write systems change work plans for selected sites with sustainability strategies; collaborate with UMAHEC Tobacco team for ongoing technical assistance.
- Coordinate with hospitality HR leadership to implement referral processes (automated/referral form) and track referrals to increase effectiveness.
- Work with Tobacco Program Manager to track cessation activity and evaluate plan effectiveness.
- Disseminate culturally and linguistically appropriate educational materials on smoking/tobacco cessation services/programs.
- Share information on the Florida Tobacco Quitline at clinical sites and community events.
Direct Services/Tobacco Cessation Delivery (50%)
- Complete tobacco cessation and HIPAA trainings.
- Register and enroll clients into cessation classes.
- Plan and conduct tobacco cessation counseling sessions and support groups at participating sites (in-person and online).
- Follow up with counseling participants to determine appropriate interventions.
- Develop, with clinical sites, an effective HIPAA-compliant system for recording cessation information in medical records.
