Summary

The Population Health Liaison plays a pivotal non‑clinical role within the JHS Population Health Department. This position is responsible for coordinating and facilitating home‑visit activities for individuals enrolled in various community‑based programs. The Liaison serves as a coach, educator, and connector – empowering individuals and families through health literacy, resource navigation, and collaborative engagement with internal teams and external partners. The incumbent works closely with case managers, social service providers, and community stakeholders to ensure that individuals receive personalized guidance and support aligned with their goals, while promoting self‑efficacy and sustained engagement in health‑promoting behaviors.

Responsibilities
  • Coordinate and monitor home‑visit activities to reinforce non‑clinical education, goal setting, and resource connection as outlined by program staff.
  • Facilitate health literacy and self‑management coaching, supporting individuals in understanding their care plans and navigating available services.
  • Serve as a resource navigator, coordinating referrals, documentation, and communication related to community benefits, social services, and support programs.
  • Collaborate with internal teams across Population Health and other departments to ensure alignment of outreach efforts and share insights from field activities to inform strategic planning.
  • Act as a facilitator and advocate, helping individuals understand their rights, access community‑based services, and engage meaningfully with available support systems.
  • Support program development, contributing ideas and feedback to initiatives that motivate individuals to participate in wellness, education, and social service programs.
  • Maintain accurate records of outreach activities, coaching sessions, and educational materials. Ensure documentation meets organizational and regulatory standards.
  • Provide feedback to case managers and stakeholders on individual engagement, barriers to participation, and opportunities for enhanced outreach strategies.
  • Build and maintain relationships with community agencies, staying informed on referral processes and available programs that align with non‑clinical support goals.
  • Participate in team meetings and training sessions, contributing to discussions on program coordination, communication strategies, and continuous improvement.
  • Support operational functions, including scheduling, tracking performance metrics, and preparing reports and presentations for leadership.
  • Serve as a liaison for interdepartmental communication, aligning Population Health initiatives with organizational priorities, grant requirements, and strategic objectives.
  • Perform other related duties as assigned to support the mission and goals of the department.
Experience

Generally requires 3 to 5 years of experience in an administrative operational function and coordination of activities. Direct experience in community health, public health, or a related field is strongly preferred.

Education

High school diploma is required;
Bachelor’s degree is strongly preferred.

Credentials

Current certification as a Community Health Worker (CHW) is required.
