Physical Therapist Sub-Acute and Outpatient
Miami Jewish Health is one of the largest providers of healthcare and living options for aging adults in the Southeast. Our four‑star rated skilled nursing and rehabilitation facility is located on a beautiful 20+ acre campus just north of mid‑town Miami.
We serve approximately 1,200 patients and residents each year, providing a full range of long‑term care, treatment, support, and post‑hospitalization services, including skilled (sub‑acute) and outpatient. Join us to do purposeful work with our diverse and respectful team.
Job SummaryEvaluates and implements physical therapy treatment plans to patients with physical impairments related to the musculoskeletal, neurological, integumentary system, and other conditions that may impact physical function. Physical therapy works to restore and/or maintain the patient’s function, alleviate pain, and prevent disabilities by planning and administering medically prescribed skilled physical therapy interventions. Collaborates with physician, family members and other members of the healthcare team.
Provides training and education to family/caregivers and other professionals.
- Completes a comprehensive physical therapy evaluation and plan of care
- Develops a comprehensive treatment plan with goals that align with identified impairments for each participant to maximize potential
- Begins discharge planning on evaluation and involves the patient, family and/or caregiver in establishing goals for rehabilitation and discharge planning, as evidenced in the documentation
- Reviews patient’s status at regular intervals and changes the treatment program as necessary
- Participates in and provides in‑service education to facilities, supportive staff, students, and the community
- Attends both departmental and IDT (interdisciplinary team) meetings
- Establishes a cooperative working relationship between the IDT, rehab team and members of the medical community
- Supervises and trains support personnel
- Documentation aligns with best practice standards and is accurate and timely
- Identifies potential problem situations, intervenes appropriately, and communicates such events to supervisor
- Performs related non‑clinical services necessary to maintain the efficient operation of the department, and adjusts schedule as required
- Demonstrates willingness to work across clinical service lines, including inpatient, outpatient, and PACE as needed
- Education: Master’s degree preferred
- Experience: One year in physical therapy and experience working with the geriatric population preferred
- Licenses/Certifications: Florida licensed physical therapist, BLS certified

Required:
Current knowledge of safety issues needed in the treatment of patients; effective written and oral communication skills; bilingual English/Spanish preferred; good problem‑solving and clinical judgment; initiative and adaptability; organization and punctuality; ability to work with diverse backgrounds; computer literacy; ability to lift 50 pounds, transfer patients up to 100 pounds safely - Functional Demands: Normal working condition with adequate lighting and ventilation; proximity to moving mechanical parts and housekeeping agents; routine exposure to blood/body fluids/tissues; physical tasks approximately 80% (lifting, turning, moving, transporting patients); sedentary tasks 20%; ability to perform minimal assist transfers; exert up to 100 pounds occasionally
