Our four-star rated skilled nursing and rehabilitation facility is located on a beautiful 20+ acre campus just north of mid-town Miami. We serve approximately 1200 patients and residents each year, providing a full range of long‑term care, treatment, support, and post‑hospitalization services.

Prevents, evaluates, treats, and diagnoses speech, language, social communication, cognitive‑communication, and swallowing disorders. Develops treatment plans that utilize appropriate interventions, strategies, and materials. Collaborates with physicians, family members, and other members of the healthcare team. Provides training and education to family/caregivers and other professionals.
Essential Job Functions- Completes a comprehensive evaluation and plan of care
- Develops an appropriate and comprehensive treatment plan for each participant to maximize potential.
- Begins discharge planning on evaluation and involves the patient, family and/or caregiver in establishing goals for rehabilitation and discharge planning, as evidenced in the documentation
- Develops an appropriate and comprehensive treatment plan for skilled therapy services that aligns with identified deficits and both short and long‑term goals
- Reviews patient’s status at regular intervals and changes the treatment program as necessary
- Monitors therapy utilization for appropriateness and alignment with regulatory requirements
- Participates in and provides in‑service education to facilities, supportive staff, students, and the community
- Attends both departmental and IDT (interdisciplinary team) meetings
- Establishes a cooperative working relationship between the IDT, rehab team and members of the medical community
- Supervises and trains support personnel
- Monitors documentation for (GG coding, progress notes, daily notes, charge capture, demographics, eval and discharge notes) accuracy and timing
- Consistently maintains 90% productivity.
- Identifies potential problem situations, intervenes appropriately, and communicates such events to supervisor
- Performs related non‑clinical services necessary to maintain the efficient operation of the speech therapy area, and adjusts schedule as required to meet the needs of participants.
- Utilizes appropriate safety measures when providing care to participants and other job duties
- Demonstrates willingness to work across clinical service lines, including inpatient, outpatient, and PACE as needed
- Master’s Degree in Speech Language Pathology from an accredited school
- One year experience in speech language pathology and experience working with the geriatric population preferred
- Florida Licensed Speech Language Pathologist
- Current BLS certification
- Certified with the American Speech‑Language‑Hearing Association (CCC‑SLP) preferred
- Current knowledge of safety issues needed in the treatment of patients, especially regarding body mechanics and body substance precautions to protect self, patients and personnel working with therapist
- Effective written and oral communication skills Bilingual English/Spanish preferred
- Good problem solving and clinical judgment skills
- Demonstrate ability to take initiative, be adaptable and work well independently
- Be well organized, flexible, dependable, and punctual
- Ability to work well with staff, individuals, and participants of diverse ethnic and cultural backgrounds
- Must be computer literate (Microsoft Office, Excel)
- Normal working condition with adequate lighting and ventilation
- Proximity to moving mechanical parts, and housekeeping/cleaning agents
- Routine exposure to blood/body fluids/tissues
