Registered Nurse Home Health – North Miami
Join Oasis Health Partners as a Registered Nurse Home Health in North Miami. Your role involves administering skilled nursing care to clients requiring professional nursing services and teaching and supervising the family and other members of the nursing team. Each registered nurse performs nursing services for residents within the Homestead and West Kendall area.
Qualifications- Graduate of an approved school of professional nursing.
- Licensed Registered Nurse in the applicable state.
- Minimum of one (1) year of experience as a Professional Nurse.
- A Baccalaureate degree is preferred, but is not required.
- Conduct the admission of the client by obtaining a medical history, particularly as it relates to the present condition, from the client and/or family members.
- Conduct a physical examination of the client, including vital signs, physical assessment, mental status, appetite and type of diet, etc.
- Evaluate the client, family members and home situation to determine what health teaching will be required.
- Evaluate the client's environment to determine what assistance will be available from family members in caring for the client.
- Determine if other services such as those of a Home Health Aide will be required and the frequency of this service.
- Interpret nursing and other services of the Agency to clients and families as a part of planning for care.
- Prepare OASIS documents and additional paperwork as may be required, and implement the nursing care plan.
- Provide skilled nursing care as outlined in the nursing care plan, including:
- Nursing services, treatments, and preventative procedures requiring substantial specialized skill and ordered by the provider.
- Initiating preventative and rehabilitative nursing procedures as appropriate for the client's care and safety.
- Observing signs and symptoms and reporting to the provider any reactions to treatments, including drugs, and changes in the client's physical or emotional condition.
- Teaching, supervising, and counseling the client and caregivers regarding the nursing care needs and other related problems of the client at home.
- Supervise and evaluate the care given by the Home Health Aide at least once every 14 days, submit evaluations to the Aide Department, and participate in conferences concerning performance.
- Chart services rendered, changes noted in the client's condition and/or family and home situation, make revisions in the nursing care plan, record supervisory visits, evaluate client care and progress, prepare paperwork for continuing care, and close charts of discharged clients.
- Prepare and present the client's record to the Clinical Record Review Committee as indicated.
- Evaluate the effectiveness of nursing service to the individual and family.
- Consult with the attending provider concerning alteration of the Plan of Care.
- Submit clinical notes no less often than weekly, and progress notes and other clinical record forms outlining the services rendered.
- Submit a tally of visits made each day or as required.
- Participate in case conferences.
- Discuss with the supervisor problems concerning the clients and how they may best be handled.
- Coordinate involvement of other members of the health team such as the Home Health Aide, Physical Therapist, Speech Therapist, Occupational Therapist, Social Worker, etc.
- Obtain orders for paramedical service and submit referral to appropriate personnel.
- Coordinate total client care and cooperate with other agencies providing nursing or related services to provide continuity of care.
- Participate in staff development meetings and the planning, operation and evaluation of the nursing service.
- Participate in the development and periodic revision of the provider's Plan of Care and process change orders as needed.
- Maintain an ongoing knowledge of current drug therapy.
- Prepare the care plan for the Home Health Aide/Personal Care Aide.

In-Person. - Job Type: Per Diem.
- Hours of Operation:
Monday through Friday (shifts as needed).
- Must have a car with required insurance coverage and a state driver's license or reliable transportation.
- Malpractice Insurance.
- Able to communicate verbally and in writing to the extent required by the position.
- Able to physically perform the duties required by the position.
- Able to travel to prospective clients' residences within Miami-Dade county.
