Core Job Functions- Collaborates and facilitates patient discharge plans for those requiring post patient care.
- Reassesses plan of care according to the physical and emotional needs of patient.
- Performs patient assessment, including looking at illness, mental status, and family dynamics.
- Documents relevant data accurately and in compliance with laws and regulations.
- Links patients and their families to appropriate community resources.
- Serves as patient advocate and liaison to physicians, insurance companies, and community agencies.
- Helps the patient and their family to understand, accept, and follow medical recommendations.
- Provides clinical supervision and guidance to junior social workers.
- Adheres to University and unit-level policies and procedures and safeguards University assets.

Core Qualifications- Master’s degree in social work
- Minimum 1 year of experience
