Position Summary
:
Clinical Hospital Pharmacist responsible for drug therapy management of assigned patients, providing clinical drug information to physicians, nurses, and patients, educating pharmacy students and staff, and distributing medications per established policies and procedures.

Duties & Responsibilities

• Evaluate medication therapy for appropriateness considering patient conditions, allergies, renal status, drug‑drug and drug‑food interactions, formulary status, dosage, and alternative therapies.
• Provide therapeutic drug level monitoring when appropriate and communicate with prescribers to optimize therapy.
• Consult with other healthcare practitioners, pharmacy staff, and management as needed.
• Identify trends, collect data, and formulate practice recommendations that improve outcomes or reduce cost.
• Document all clinical consults in the medical record following hospital guidelines.
• Demonstrate accuracy and efficiency in dispensing, preparing, labeling, and data entry.
• Contribute to quality improvement, safety initiatives and clinical research.
• Educate pharmacy students, interns, residents, nurses, and other staff.
• Participate in multidisciplinary committees, serve as secretary or chairperson, and support administrative functions.
• Maintain proficiency with area‑specific technology (automated dispensing machines, IV setups, pre‑packaging, etc.) and troubleshoot issues.
• Ensure compliance with required documentation activities such as adverse drug reactions, clinical pharmacy interventions, incident reports, and medication errors.
• Perform other related duties as needed and demonstrate service excellence aligning with CARE values (Compassion, Accountability, Respect, Expertise).

Required Experience

• 5–7 years of related experience or the equivalent in a clinical pharmacy setting.
• Pharm.
  
  D. degree and residency training or equivalent proven experience.
• Florida pharmacist license or eligibility to obtain one.

• Strong analytical, organizational, and prioritization skills with the ability to meet multiple deadlines.

• Proficiency in medication management and clinical decision‑making.
• Commitment to continuous learning and professional development.

Additional Credentials

• Possession of or eligibility for Florida pharmacist license.
