Position Summary- Candidates must be located in Miami Dade County, in the surrounding Kendall, Cutler Bay, Homestead area (Zip Codes: 33184, 33172, 33186).
- This role will require 50‑75% travel for face‑to‑face visits with members in Miami Dade.
- Schedule is Monday‑Friday, 8:00 am‑5:00 pm, standard business hours.
We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our Case Management team. This opportunity offers a competitive salary and full benefits. Responsibilities- Telephonically and/or face‑to‑face assess, plan, implement, and coordinate all case management activities with members to evaluate their medical needs and facilitate overall wellness.
- Manage a caseload that includes supportive and medically complex members.
- Develop a proactive course of action to address issues presented and enhance short and long‑term outcomes.
- Determine appropriate services and supports due to member’s health needs, including prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, and community resources.
- Bilingual in English and Spanish.
- Must reside in Kendall, FL (Zip Codes: 33184, 33172, 33186).
- Willing and able to travel 50‑75% of the time to meet members face to face in Miami Dade County and surrounding areas.
- 1+ year of experience in behavioral health or a long‑term care setting.
- Ability to multitask, prioritize, and adapt to a fast‑paced changing environment.
- Effective verbal and written communication skills.
- Managed care experience.
- Computer proficiency in Microsoft Word, Excel, and Outlook.
- Case management and discharge planning experience.
- Bachelor’s degree required, preferably in social work or a related field.

Pay RangeTypical pay range: $21.10 – $36.78 per hour. This range represents the base hourly rate or base annual full‑time salary for all positions in the job grade. The actual base salary offer will depend on experience, education, geography and other factors.
