This Bilingual Analyst, Case Management position is with Aetna’s Long‑Term Services and Supports (LTSS) team and is a field‑based position out of North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL. The role requires traveling 50‑75% of the time to meet members face‑to‑face.

Schedule:

Monday‑Friday, 8:00 am‑5:00 pm.

We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our team. This opportunity offers competitive salary and full benefits. The coordinator will telephonically and/or face‑to‑face assess, plan, implement, and coordinate all case management activities with members to evaluate and meet their medical needs and facilitate overall wellness.

• Manage a caseload of supportive and medically complex members.
• Develop proactive plans to address issues and enhance short‑ and long‑term outcomes.
• Determine appropriate services and supports such as prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, and community resources.

• Bi‑lingual in English and Spanish.
• 1+ year of experience in behavioral health or a long‑term care setting.
• Computer proficiency in Microsoft Word, Excel, and Outlook.
• Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL.
• Must possess reliable transportation and be willing and able to travel up to 50‑75% of the time.

• Ability to multitask, prioritize, and adapt effectively in a fast‑paced, changing environment.
• Effective verbal and written communication skills and managed‑care experience.
• Case‑management and discharge‑planning experience.

• Bachelor’s degree required, preferably in Social Work or related field.
• Master’s degree preferred.

Pay Range: $21.10 – $36.78 per hour (base rate; actual offer depends on experience, education, and geography). Eligible for CVS Health bonus, commission, or short‑term incentive programs.
