Position
Description:
The Primary Care Registered Nurse (RN) delivers high-quality, patient-centered care across diverse clinical settings, including Primary Care/Patient Aligned Care Teams (PACT), Medical Specialty Clinics, Cardiac Catheterization/Electrophysiology Laboratory (CCL), and Gastroenterology (GI) services. This role supports Veterans in a multidisciplinary outpatient environment by combining clinical expertise, care coordination, patient education, and procedural assistance.
- Act as the care coordinator for the PACT in an outpatient setting.
- Check-in patients, perform disease management, scrub appointments, and increase patient access through My Healthy Vet /secure message responses.
- Collaborate with community care and perform duties as assigned.
- Conduct patient assessments and provide medication administration and IV therapy.
- Coordinate interdisciplinary team activities, telemetry monitoring, admissions, and discharges.
- Manage internal and external transfers and delegate tasks to assistive personnel as appropriate.
- Document in electronic medical records and coordinate patient transport.
- Respond to emergencies in a timely manner.
- Maintain at least two (2) years of nursing experience in an outpatient setting.
Minimum Qualifications:
- Primary Care RN shall have BLS certification and at least two (2) years of inpatient experience in care coordination.
- Medical Specialty RN shall have ACLS and BLS and three (3) years of experience in an outpatient setting (e.g., Telemetry and Acute Care).
- The personnel assigned to perform the services shall possess an unrestricted license to practice medicine in one of the States, Territories, or Commonwealths of the United States or the District of Columbia.
