Position SummaryThis Bilingual Analyst, Case Management position is with Aetna’s Long-Term Services and Supports (LTSS) team and is field-based with travel 50-75% on schedule Monday-Friday 8:00am-5:00pm. The role requires self-motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to assess, plan, implement, and coordinate case management activities with members to evaluate medical needs and facilitate wellness through integration of prior authorization, coordination with PCP, condition management, medication review, and community resources.
Responsibilities- Telephonically and/or face‑to‑face assessing, planning, implementing, and coordinating all case management activities with members.
- Effectively manage a caseload that includes supportive and medically complex members.
- Develop proactive courses of action to address issues presented to enhance short‑ and long‑term outcomes.
- Determine appropriate services and supports due to member’s health needs, including but not limited to prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, and community resources.
- Bi‑lingual in English and Spanish.
- 1+ year of experience in behavioral health or a long‑term care setting.
- Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL.
- Reliable transportation and willing to travel up to 50‑75% of time to meet members face‑to‑face; mileage reimbursed per company policy.
- Ability to multitask, prioritize, and effectively adapt to a fast‑paced changing environment.
- Effective communication skills, both verbal and written.
- Managed Care experience.
- Computer proficiency in Microsoft Word, Excel, and Outlook; case management and discharge planning experience.
- Bachelor’s degree required, preferably in Social Work or a related field.
- Master’s degree preferred.

Pay RangeBase hourly rate: $21.10 – $36.78 per hour. Additional incentive programs may apply.
