Summary

Clinical Hospital Pharmacist by using established policies and procedures is responsible for: drug therapy of assigned patients; provision of clinical drug information to meet the needs of the physician, nurse, and patient, education of pharmacy students, residents, and interns; and distribution of medications.

Responsibilities
  • Evaluates medication therapy for appropriateness, considering patient conditions (age, allergies, renal status, comorbidities), safety and potential efficacy of prescribed medications (identity, strength, dose, frequency, duration, directions), formulary status, potential drug interactions, and alternatives (cost or therapeutic value).
  • Provides therapeutic drug level monitoring when appropriate.
  • Communicates effectively with nurses and prescribers when errors are detected or to optimize therapy.
  • Consults with other practitioners (other Clinical Hospital Pharmacists, Pharmacy Management, Attending Physician, etc.) as needed.
  • Identifies trends in pharmaceutical care, collects data, and formulates recommendations to improve outcomes or reduce costs.
  • Presents data to the appropriate committee as required.
  • Maintains exceptional feedback from medical staff regarding pharmacist impact on patient outcomes.
  • Performs formal, written consults and documents all clinical consult information in the medical record per hospital guidelines.
  • Demonstrates accuracy in dispensing, preparation, labeling, and data entry; supports safe use of automated dispensing systems.
  • Identifies ways to reduce medication dispensing errors.
  • Completes tasks efficiently without compromising accuracy.
  • Assists supervisors by suggesting procedural, program, or system improvements to enhance service, patient outcomes, and safety while reducing costs and protecting assets.
  • Develops the skills of others; may be responsible for orientation, staff development, and education/training of pharmacy students, interns, residents, and other healthcare personnel.
  • Helps improve performance of new and junior staff through coaching; meets area-specific quantitative requirements; evaluations of task quality are consistently satisfactory.
  • Volunteers for additional teaching assignments and evaluations; consistently offers suggestions to improve training programs.
  • Identifies coaching needs for junior pharmacists and technicians with documented recommendations to the supervisor.
  • Makes at least one presentation per year at local or national meetings or publishes in an internal/external publication at least every other year; conducts journal clubs or in-service presentations annually.
Experience

Generally requires 5 to 7 years of related experience.

Education

Pharm.

D. degree and residency training or equivalent proven experience is required.

Credentials

Possession of or eligible for Florida pharmacist license is required.
