Position Summary: We are seeking a compassionate and motivated Community Health Worker to support a caseload of up to 30 individuals living with chronic health conditions. This role is primarily community-based and focuses on helping patients improve their overall health and well-being through ongoing engagement, education, and care coordination. In this position, you will work directly with patients providing guidance, resources, and support tailored to their unique health needs.
You will also collaborate closely with medical providers, primary care teams, behavioral health specialists, and dental professionals to ensure patients receive comprehensive, coordinated care that leads to better health outcomes.
- Utilize the PRAPARE tool to assess Social Determinants of Health (SDOH) that may impact a patient’s health status.
- Build productive, supportive relationships with patients to address SDOH needs and promote improved health outcomes.
- Develop individualized treatment plans and document progress toward identified goals.
- Review and update treatment plans quarterly, collaborating with the treatment team to make adjustments as needed.
- Engage natural supports (family, caregivers, community members) in the patient’s treatment process.
- Assist patients with accessing and using available resources within the center and the community, including help with scheduling appointments and monitoring referral completion.
- Maintain timely, accurate, and high-quality documentation.
- Demonstrate strong time-management skills, including punctuality for appointments, participation in team meetings, and completion of documentation on schedule.
- Work to improve the overall health status of individuals served.
- Effectively prioritize workload in a dynamic environment.
- Collaborate as a key member of an integrated care team.
- Partner with the Quality Improvement Team and support ongoing PDSA quality improvement projects.
- Maintain the ability to sit and drive for a significant portion of the workday.
- Strong communication and interpersonal skills
- Basic computer proficiency and ability to document in EHR systems
- Ability to work independently and prioritize multiple tasks
- Critical thinking and problem-solving skills
- General understanding of mental health, Medicaid, Marketplace, and insurance benefits
Meets at least one of the following:
- Meets the criteria to bill as a community support.
- Has a bachelor’s degree in social work, psychology, or closely related fields or is a licensed nurse.
- Equivalent experience may be substituted based on one year of experience for each year of required education.
