• Predictable weekday schedule — no nights or weekends
• Supportive, team-based environment
• Meaningful work serving older adults in the community

• Deliver comprehensive primary care to older adults
• Manage acute and chronic conditions
• Complete home and facility visits as needed
• Educate participants and document care accurately
• Participates in the after-hours on-call coverage rotation

• Licensed NP or PA in Massachusetts
• Geriatric or primary care experience preferred
• Strong clinical skills, communication, and teamwork
