We’re seeking a Registered Nurse Care Manager to join a dynamic care management team dedicated to supporting people with complex and chronic conditions. Many of our patients face not only health challenges, but also social and economic barriers — and this role is your chance to be the link that guides them toward better outcomes and a healthier future.
What You’ll Do- Conduct comprehensive assessments and develop tailored care plans
- Use motivational interviewing to engage and empower patients
- Coordinate care across multiple settings and providers
- Provide education, medication reconciliation, and discharge planning support
- Connect patients to primary care, behavioral health, social services, and community resources
- Collaborate closely with an interdisciplinary team to remove barriers to care
- Hybrid model — flexibility to work remotely and in the community
- Regular team meetings at local practice locations
- Opportunities to directly impact patients across diverse care settings
- Active MA RN license (LPN with strong Care Management background considered)
- 3–5 years of nursing experience (home health, ambulatory care, community/public health, or case management ideal)
- Bilingual in Spanish strongly preferred
- Valid driver’s license
