Position SummaryGLFHC is seeking a CSS Social Services Coordinator to support the Community Support Services (CSS) Department. This role is responsible for coordinating referrals, pre-visit planning, no-show monitoring, and performance reporting to improve patient access, service delivery, and program effectiveness. The Coordinator plays a key role in communication, data tracking, and operational support across departments.
Key Responsibilities Referral Coordination- Receive, log, and track referrals to CSS.
- Communicate with referring providers, patients, and staff.
- Prepare referral status reports for meetings.
- Coordinate pre-visit planning and required documentation.
- Track completion of pre-visit requirements.
- Address missing information as needed.
- Track patient attendance for CSS services.
- Prepare no-show reports and identify trends.
- Support strategies to reduce missed appointments.
- Collect and analyze referral, attendance, and outcome data.
- Prepare monthly, quarterly, and ad-hoc reports.
- Support quality improvement and compliance efforts.
- Prepare onboarding materials and system access.
- Support training on workflows and procedures.
- Assist with scheduling, meetings, and documentation.
- Maintain accurate records in EHR and program databases.
- Support communication across CSS and other departments.
- Experience Minimum 2 years in healthcare, community services, or administrative support preferred; strong organizational and data management skills; proficiency in Microsoft Excel, Word, and Outlook; experience with EHR systems preferred; strong verbal and written communication skills; valid Massachusetts Driver’s License and reliable transportation.
- Education & Certifications Degree in Human Services, Public Health, Healthcare Administration, or related field preferred; combination of education and experience considered; bilingual/bicultural (Spanish/English) strongly preferred;
Medical Interpreter Certification (or completion within 12 months);
Community Health Worker Certificate (or completion within 12–18 months).
