Position SummaryThe LPN is responsible for assessing the medical needs of potential and current clients, ensuring that services provided are appropriate and meet their healthcare requirements.
Schedule:
Monday - Friday, 8:30am-5pm. This role will be a mix of in office and home visits.
- Develop and update comprehensive care plans.
- Provide staff training, ensure accurate documentation, and support medication management.
- Conduct in-home health assessments, administer IM and SC injections, and address medical concerns.
- Supervise caregivers for proper medication administration, vital sign documentation, and protocol compliance.
- Manage client medications, coordinate refills, and liaise with pharmacists.
- Lead caregiver training and evaluate competency in essential medical tasks.
- Maintain accurate health records and electronic documentation.
- Communicate effectively with families, caregivers, and team members regarding client needs and updates.
- Work under RN supervision while operating independently in the field.

- Licensure:
Valid State LPN License. - Compact license preferred.
- Education:
High school diploma or GED required. - Experience:
Minimum 1 year in home care, healthcare, or senior-related industry (preferred). - Technical
Skills:
Proficient with software tools such as Adobe, Outlook, Teams, Word; ability to learn new EMR systems. - Communication:
Excellent written and verbal communication skills. - Training Ability:
Comfortable leading caregiver training sessions for groups of 5–10. - Transportation:
Reliable and consistent transportation for travel within assigned territory. - Core Competencies:
