Primary Care Nurse Practitioner / Physician Assistant opportunity in Mesa, AZ

• 20 to 30 Patients per day
• Outpatient primary care (no inpatient)
• Strong leadership and team-based culture
• Well-respected, expanding organization
• Meaningful work improving community health outcomes
• Manage acute and chronic conditions
• Provide preventive care, wellness visits, and patient education
• Perform assessments, diagnostics, and evidence-based treatment plans
• Collaborate with physicians, MAs, RNs, and care coordination teams

• Active Arizona NP or PA license
• NP/PA Program Completed in USA (Not Puerto Rico)
• Primary care or outpatient experience preferred
• Passion for compassionate, patient-centered care
