Responsibilities
- Assess, diagnose, and treat individuals and families with mental health or substance use disorders, including schizophrenia, depression, bipolar, and personality disorders.
- Coordinate medication management, including bubble packs, injections, and hospital discharge medications.
- Update and manage a database tracker for hundreds of patients’ injections, Clozaril labs, and annual labs.
- Conduct triages, refer patients to emergency care or appropriate agencies, and provide home visits per ACT Criteria.
- Administer injectable medications, lab draws, and other treatments per physician orders.
- Document assessments and plans in the Next Gen database.
- Mentor new nurses and medical assistants, ensuring they receive comprehensive onboarding and continuous professional development.
- Participate in ICC meetings with medical teams to discuss high-risk patients and care coordination.
- Travel:
Position is based out of Mesa, AZ HQ, but travel will be required to visit patients across the greater Metro Phoenix area.
- Education
:
Bachelor’s Degree in Nursing, or closely related.
- Experience
: 6 months of experience in job offered, Cardiac Progressive Care, Immunization Nurse, or closely related.
- Special skills
:- Assess, plan, implement, and evaluate the psychological well-being of postoperative patients, including identifying and addressing signs of anxiety, agitation, or mood disorders.
- Assist in the administration of injectable medications while monitoring for any reactions or complications.
- Provide support to patients and their families about the emotional and psychological aspects of medical treatments and care.
- Coordinate care between interdisciplinary teams, ensuring communication addresses the psychological needs of patients, particularly those with mood disorders or agitation.
- Ensure and uphold sterile environments during procedures, including proper sterilization of equipment and adherence to infection control protocols.
- Provide mentorship and training to new nurses and medical assistants, ensuring they are well-prepared for patient care responsibilities.
- Must have a valid Registered Nurse License for the state of Arizona.
