Job type: Part‑time (2–10 hours per week, Thursday/Friday)
Salary: $57,000–$64,000 per year (MSN: $57K–$59K DOE; DNP: $64K). Additional language differential pay available for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili, ASL: $3/hour ($6,240 annually).

Job summary: The Primary Care Provider conducts physicals/check‑ups, medical evaluations, diagnostics, treats illnesses, orders lab tests, provides general and preventative care, prescribes and monitors medications, coordinates with other providers, makes specialty care referrals, educates patients about preventative care, and establishes therapeutic relationships for treatment goals within an integrated setting.
Duties:
- Performs independently or as part of a treatment team.
- Conducts physicals/check‑ups, evaluations, diagnostics, treats illnesses, orders lab tests, and provides general and preventative care.
- Prescribes and monitors medications, including individual treatment and case consultation.
- Provides patient education about preventative care, wellness activities, and prescribed treatments.
- Provides leadership and cohesion within the clinic environment; may supervise other professional and non‑professional medical staff and students.
- Participates in program evaluation procedures, professional record‑keeping, and appropriate referrals for specialty and behavioral health care.
- Provides direct patient services not less than 85% of the work time.
- Employs therapeutic interventions appropriate for identified treatment issues.
- Seeks appropriate clinical supervision/support when necessary to ensure effective treatment.
- Establishes appropriate patient files that conform to agency guidelines, best professional practice, and State licensing regulations.
Qualifications:
- Active AZ Family Nurse Practitioner (FNP) license.
- Certified Family Nurse Practitioner (C‑FNP) or equivalent.
- Preferred experience:
FNP practice;
Next Gen EMR experience a plus. - Strong clinical judgment and ability to work independently in an integrated setting.
