POSITION SUMMARY This position is accountable for the quality of rehabilitative services delivered by self and through the supervision of team members who are under their direction. This position utilizes specialized knowledge, judgment, and rehabilitation skills necessary to assess data and plan, provide and evaluate care. Manages personnel to ensure appropriate allocation of resources in providing quality patient care and appropriate rehabilitation care of patients in accordance with organizational policies, regulatory bodies, and leadership direction.
CORE FUNCTIONS
1. Provides, facilitates and supervises patient centered care and care coordination to ensure patient care needs are met while maintaining quality standards and regulatory compliance in a financially responsible manner. Performs work with minimal supervision. Assures efficient operations to meet customer needs. Internal customers include physicians, staff, and management. External customers include patients, families, and surrounding community.
2. Serves as a resource to patients, families, physicians, and staff by providing hands on care, clinical support, managerial support, interpreting policies, and facilitating care. Provides departmental leadership by demonstrating effective communication skills, problem solving, and proactive conflict resolution to internal and external customers in situations.
3. Hires, leads, mentors, develops, and evaluates staff in a manner that promotes engagement, productivity, and ensures competencies. Mentors staff to increase clinical skills, critical thinking, problem solving skills and promote retention. Conducts individual and team goal setting, performance planning and evaluation. Supports staff in adoption of change and participates in the development, interpretation, implementation and evaluation of process improvement and quality management activities.
4. Supervises and delivers care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors.
