This is a full-time, on-site role located in Mesa, AZ, for a Patient Care Coordinator. The Patient Care Coordinator will oversee and manage efficient appointment scheduling, provide exemplary care coordination, and communicate effectively with patients and healthcare providers. Day-to-day responsibilities include coordinating patient care plans, ensuring smooth communication between parties, and leveraging medical knowledge to assist patients and clinical teams. Strong organizational skills are essential to manage multiple tasks and deliver high-quality patient care services.
Qualifications- Expertise in Appointment Scheduling and adherence to proper Phone Etiquette
- Proficiency in Care Coordination and Patient Care principles
- Working knowledge of Medical Terminology to support patient and clinical interactions
- Strong interpersonal and organizational skills
- Ability to work effectively in a fast-paced, on-site team environment
- Experience in a healthcare or clinical setting is a plus
- High school diploma or equivalent; additional certifications or relevant training are an advantage
