SMI Navigator – Case Management

The Esperanza team is searching for our new SMI Navigator who is ready to make an impact with their case management and SMI experience.

SCHEDULE
Monday—Friday, 8 am–5 pm. The Esperanza team meets clients in office and in the community.

325 N. Stapley Dr., Mesa, AZ

ABOUT Esperanza
CPLC‑CARF accredited Centro Esperanza offers specialized services for SMI populations focusing on case management and psychiatric services. We provide trauma‑informed support tailored to individual needs, supporting the people we serve on their recovery journey.

HOW YOU WILL MAKE AN IMPACT

As the SMI Navigator, you will be an integral part of an interdisciplinary team providing case management services to a caseload of severely mentally ill adults with or without co‑occurring substance abuse disorders. You act as the advocate and support for each client, engaging with community agencies, health‑care providers, and others on their behalf to ensure access to the services needed to increase wellness, self‑management, and reduce emergency‑room visits and/or hospitalizations.

You provide clinical support, consultation, education, and information around psychosocial and/or substance‑abuse conditions, interventions, and resources to maintain focus on outcomes and best practices, participating in assessments and service plan development.

  • Guide clients through comprehensive care: participate in assessments and service planning, ensuring every client’s needs are identified and addressed with individualized strategies.
  • Coordinate services across multiple systems: connect clients with behavioral health, medical, dental, social, and community resources.
  • Provide outreach and crisis support: follow up on missed appointments, respond to crises, and maintain ongoing engagement.
  • Drive and transport clients safely: provide services to clients in the community and transport them in agency or personal vehicles as needed.
  • Maintain accurate records and monitor progress: document all activities, track treatment effectiveness, and adjust service plans.

  • You communicate with clarity and compassion: you advocate for clients and build trust while navigating complex systems.
MINIMUM QUALIFICATIONS
  • Master’s degree in a behavioral health related field.
  • Bachelor’s degree in behavioral health related field (required); one year behavioral health work experience preferred.
  • Master’s degree in a non‑behavioral health related field with 30 semester hours in behavioral health education and two years behavioral health work experience.
  • Bachelor’s degree in a non‑behavioral health related field with 30 semester hours in behavioral health education and two years behavioral health work experience.
  • Associate’s degree in behavioral health related field with four years behavioral health work experience.
  • Associate’s degree in a non‑behavioral health related field with 30 semester hours in behavioral health education and five years behavioral health work experience.
  • High school diploma or equivalent with 18 credit hours of post‑high school education in a field related to behavioral health (completed no more than 4 years before date of proposed employment) and two years full‑time behavioral health experience; or four years behavioral health work experience.
