Wellness Navigator

Job SummaryThe Wellness Navigator will help to remove personal and environmental obstacles to health care access. The Wellness Navigator is responsible for working closely with the primary medical provider, the clinical team, and staff of other agencies involved in the patient’s care to proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy and support primarily in-home and/or in the community.
The Wellness Navigator will engage, educate and offer support to individuals, their family, patients, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up.

Skills/Requirements- High school diploma or GED required. An associate degree (or higher) in a field related to behavioral health is preferred.
- Education and/or experience in the field of health and nutrition preferred.
- Or equivalent combination of education and experience.
- Current AZ Driver’s License.
- Clean 39-month MVR.
- Behavioral Health Technician (BHT) in accordance with A.A.C. R and CBI Clinical Policies and Procedures required.
- Arizona Fingerprint Clearance Card required.
