SCHEDULEThis is on-call Monday‑Friday during business hours, 8 am‑6 pm. Hours may vary based on need and availability. The role is field‑based – working in office and in the community.
About EsperanzaEsperanza offers specialized services for Serious Mental Illness (SMI) populations focusing on case management and psychiatric services. SMI Case Managers meet clients where they are most comfortable and able to receive services, in office or in the community.
HOW WILL YOU MAKE AN IMPACT?Under the direction of the Clinical Coordinator, you are going to coordinate, monitor, and provide direct services to members and families at the clinic and in the community, including case management, rehabilitation, support, and crisis intervention services. You will collaborate often in a multi‑disciplinary team setting to develop individual service plans (ISPs) for individuals with a serious mental illness and to provide or coordinate all services identified.
As the SMI Case Manager, you are committed to meeting members where they are and providing services in the office, in the community, at their place of employment, and at home as appropriate.
Must meet minimum monthly productivity standards.
- Coordinate care across multiple providers, working with families, behavioural health, medical, and community resources to ensure every service in the individualized plan is delivered.
- Provide hands‑on support, from crisis intervention to resource navigation, assisting clients in maintaining and improving their behavioural health services and daily functioning.
- Manage and monitor service plans by developing, implementing, and updating individualized service plans, ensuring continuity of care and smooth transitions between levels of service.
- Handle case management essentials such as conducting assessments, maintaining accurate clinical records, screening for financial entitlements, and facilitating referrals to community resources.
- Engage in outreach and follow‑up, tracking missed appointments and participating in case conferences to keep clients connected and supported.
- Be active in the community, meeting clients where they are most able to receive services, providing transportation as needed.
To be considered a qualified behavioural health technician or a behavioural health paraprofessional, a person must have one of the following combinations of education, license and/or behavioural health work experience:
Behavioural health technician (BHT)- Bachelor’s degree in behavioural health‑related field required. One year behavioural health work experience preferred, or
- Bachelor’s degree in non‑behavioural health related field and 30 semester hours in behavioural health education and two years behavioural health work experience, or
- Associate’s degree in behavioural health related field and four years behavioural health work experience, or
- Associate’s degree in non‑behavioural health related field and 30 semester hours in behavioural health education and five years behavioural health work experience, or
- Has a high school diploma or equivalent and 18 credit hours of post‑high school education in a field related to behavioural health completed no more than 4 years before the date of proposed employment and 2 years full‑time behavioural health experience OR 4 years of behavioural health work experience.
- An associate degree, high school diploma or high school equivalency diploma.
- And meets one of the following criteria:
- Six weeks of behavioural health work experience in any setting (jails, hospitals, clinics, schools, etc.) or
- Has experienced six weeks of continuous on‑site direction from a behavioural health professional, a behavioural health technician, or a behavioural health paraprofessional who has at least six months of behavioural health work experience.
- Fingerprint clearance card
- Valid driver’s license and ability to travel throughout the community
- Ability to obtain CPR/First Aid
