PRN – Registered Nurse (RN) VISITS – Merrillville, IN Based – VARIOUS LOCATIONS

• Hours:
  
  PRN

• OASIS / MEDICARE EXPERIENCE PREFERRED
• TRAVELING TO VARIOUS LOCATIONS

• Perform skilled nursing visits including assessments, wound care, medication management, and patient education
• Complete OASIS assessments, recertifications, resumption of care, and discharges as assigned
• Collaborate with physicians, care managers, and interdisciplinary team members
• Document visits and patient progress accurately and timely in the EMR
• Educate patients and caregivers on disease processes, medications, and safety
• Adhere to infection control, safety, and compliance standards

• Current, unencumbered Indiana Registered (RN) license
• Minimum of one (1) year of clinical nursing experience; home health experience preferred
• Knowledge of home health regulations and OASIS preferred
• Strong assessment, communication, and time-management skills
• Reliable transportation and valid driver’s license
• Ability to work independently in the field
