Position Summary
Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including physicians, nurse practitioners, physician assistants, medical assistants and in‑house support staff.
We are seeking a Licensed Clinical Social Worker (LCSW), Licensed Independent Social Worker (LISW), Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Mental Health Counselor (LCMHC) or Licensed Marriage and Family Therapist (LMFT) to join our team. You will provide recovery‑oriented rehabilitation services in group or individual settings for residents in long‑term care. You will address a wide range of problems from anxiety and depression to severe and persistent mental illness and issues around death and dying, and you will play a critical role as part of a multidisciplinary team.
Essential Duties and Responsibilities- Assess each resident with valid consent to determine individual needs, develop and follow a treatment plan, and ensure that all treatment provided is medically necessary.
- Perform written or verbal consultation with the resident’s primary care physician, family, facility staff, and Eventus Whole Health support staff as needed.
- Document treatment plans, psychotherapy sessions, and updates in accordance with Eventus Whole Health procedures and policies, and provide the facility with written documentation within 48 hours of service for inclusion in the resident’s medical record.
- Meet with the Resident Care Coordinator or other designated staff upon arrival to and departure from the facility to review current care needs and communicate findings or concerns.
- Establish and maintain open and positive communication with facility staff and administration, providing verbal and/or written instruction or feedback regarding treatment plans and other pertinent caregiver information.
- Adhere to Eventus Whole Health standards, policies, and procedures, and comply with all legal, regulatory, and ethical guidelines governing healthcare delivery.
- Conduct yourself in a professional manner at all times.
- Maintain patient confidentiality at all times, including appropriate use of cell phone, email, text messaging, patient records, and EMR. Abide by HIPAA rules and regulations.
- Consult with the clinical supervisor when facing an unfamiliar resident, family, or facility request or recommendation regarding the care of a resident.
- Psychologists:
Doctoral degree from an accredited institution and a current state license as a doctoral‑level psychologist and health services provider. - Therapists:
Current state license as LCSW, LISW, LPCC, LCMHC, or LMFT, Master’s degree in social work, and current professional license.
- Assessment techniques, psychotherapy techniques, population‑specific interventions, and delivery of care following a treatment plan.
- Use of diagnostic tools and techniques, including the current edition of the DSM.
- Rules and regulations governing psychotherapy practice.
- Organization’s policies and procedures.
- Gathering and analyzing physiological, socioeconomic, behavioral, and emotional patient data.
- Accurately evaluating patient problems and providing appropriate advice, intervention, or referral.
- Written and verbal communication.
- High initiative, judgment, and discretion.
- Establishing and maintaining effective working relationships.
- React calmly and effectively in emergency situations.
- Work collaboratively with all members of the healthcare team.
- Make responsible decisions within the scope of a dependent medical practitioner.
- Evaluate and recommend continuous quality improvement.
- Handle confidential and sensitive information.
