Base pay: $70,500 – $96,900 per year (actual pay based on experience and skills; contact recruiter for details).

Pay per visit: $45.00 – $63.00 per visit/unit.

Scheduled weekly hours: 40.

• Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow‑up as directed by the Clinical Manager.
• Confer with physicians in developing the initial plan of treatment based on orders and initial assessment; provide hands‑on care, manage and evaluate the care plan, and teach patients under Clinical Manager supervision. Revise the plan in consultation with physicians based on ongoing assessments and policy/regulation.
• Coordinate appropriate care, including collaboration with physical therapists, occupational therapists, home health aides, and external providers.
• Report patient care, condition, and progress to the physician and Clinical Manager on a continuous basis.
• Implement the patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs and problems.
• Coordinate, oversee, and supervise the work of home health aides, certified home health aides, and personal care workers; provide written instructions and care plans that reflect the current plan of care; monitor documentation completion by aides as part of supervisory responsibilities.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing required clinical documentation.
• Prepare appropriate medical documentation for all patients, including case conferences, contacts, medication order changes, re‑certifications, progress updates, and care plan changes; prepare visit/shift reports, summarize patient records, and confer with other health‑care disciplines to provide optimal care.

• Diploma, Associate, or Bachelor Degree in Nursing.
• Minimum of one year of nursing experience preferred.
• Strong medical‑surgery, ICU, ER, acute experience.
• Home health experience a plus.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.
