Our company is searching for experienced candidates for the position of

• Responsible for coordination of Start of Care, Re-certification, Discharge, and Resumption of care
• Provide direct patient care, including administering medications, performing diagnostic tests, and monitoring patient vital signs.
• Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
• Educate patients and their families about medical conditions, treatment options, and preventive care measures.
• Assist with discharge planning and coordinate post-discharge care for patients.
• Provide specialized care for patients with dementia or other cognitive impairments.

• Valid Registered Nurse (RN) Required in the state of Idaho.
• Minimum 2 years home care experience
• Effective written and verbal communication
• Experience with policy, rules, and regulations
• Clean background check and driving record

$50.00 per hour + mileage
