Position Summary

Under the direction of a Registered Nurse or designated team leader, the Lead LPN provides skilled nursing care to patients in their homes. This role supports a manageable daily caseload and offers the opportunity to work independently while remaining closely connected to a supportive, interdisciplinary care team.

* Average caseload of 4-5 patients per day

* Occasional weekend coverage required

* Schedule:

8:30 a.m.

- 5:00 p.m.

Responsibilities

* Provide direct nursing care to home health patients in accordance with established care plans

* Serve as a clinical resource and support to the home health care team

* Communicate clearly and effectively with patients, families, providers, and care team members

* Collaborate with Registered Nurses and other disciplines to ensure coordinated, patient-centered care

* Support team members as needed to ensure continuity and quality of care

Qualifications

* Current Licensed Practical Nurse (LPN) license in the state of Idaho

* Experience in home health, long-term care, or acute care preferred

* Strong communication skills and a collaborative, team-oriented approach

* Ability to work independently in a home-based setting
