Overview
Part‑time and grant funded opportunity Monday, Tuesday, Wednesday, Friday 8am – 5pm. During Spring and Summer includes one Saturday a month. During Fall and Winter includes one Sunday a month. Must have reliable transportation to travel between Meriden and Middletown. The Center for Key Populations (CKP) is a program of Community Health Center (CHC), Connecticut's largest federally qualified health center, where "health care is a right, not a privilege."
CKP aims to remove barriers to comprehensive, respectful, evidence‑based care for key populations, collaborating with CHC across mobile health, quality improvement, training and education, and research to transform lives.
The daily role involves providing clinical and technical support for primary care providers, implementing planned care items with electronic tools to ensure efficient workflow, monitoring equipment and performing clinical testing per OSHA and CLIA standards, liaising with patients and the health care team, and ensuring continuity and coordination of care in a patient‑centered medical home.
Role and Responsibilities- Apply evidence‑based guidelines from major regulatory and specialty groups; perform pre‑visit preparation for all primary care patients scheduled to be seen.
- Collaborate with the primary care nurse and provider to run "e‑huddles" using the Planned Care Dashboard to ensure complete and thorough primary care visits.
- Prepare patients to be seen in the telehealth environment, guiding use of video telehealth technology.
- Manage virtual telehealth sessions for multiple providers using Zoom, including setting up virtual exam rooms, admitting patients, and pre‑visit preparation.
- Provide administrative support to clinical teams: collect paperwork, request records, schedule follow‑up visits, and coordinate other tasks.
- Facilitate telehealth patient flow by collaborating with providers, clinical and clerical staff.
- Prepare patients for visits using established criteria based on visit type (e.g., sick, PPE, well child visit).
- Accurately document chief complaints and results from evidence‑based screening tests; document patient vital signs.
- Independently perform CLIA‑waived tests, EKGs, and hearing & vision tests.
- Prioritize daily responsibilities to reduce wait times; assist team members as needed; schedule tests and referrals promptly per provider orders.
- Maintain a clean, safe work environment; oversee supplies and medical logs; stock and sanitize exam rooms daily.
- Adhere to infection control standards, universal precautions, and agency safety policies; maintain CPR certification as required for clinical staff.
- Exhibit excellent customer service with internal and external customers, focusing on quality of care and teamwork aligned with the agency mission.
- Comply with mandated in‑service training (EOC, Working Safely, Abuse, etc.).
Required Skills and Education
- High school diploma or equivalency
- Completion of a Medical Assistant course from an accredited/recognized school
- Medical Assistant board certification/registration, required or obtained within 90 days of hire
- EKG use, appointment scheduling, word processing and computer skills, and familiarity with medical testing procedures
- Strong organizational skills and accuracy
- Excellent communication and customer service skills
- Team oriented with leadership qualities
- Current American Red Cross CPR for the Professional Rescuer and AED or American Heart Association BLS/AED for Healthcare Providers certification (as a condition of employment)

- Access to clients' charts and health information; maintain client confidentiality
