Overview
Compassionate Care Hospice seeks a Hospice Chaplain to provide spiritual care to patients and families/caregivers, either directly or through coordination with other spiritual counselors. This role supports the Central California region, primarily Merced, Mariposa, and surrounding areas.

Responsibilities- Provide direct spiritual care to patients and families/caregivers.
- Assess spiritual needs of patients and families/caregivers and coordinate timely services.
- Facilitate development of the plan of care by participating in comprehensive spiritual assessments.
- Serve as liaison and support to community chaplains and spiritual counselors.
- Maintain records of spiritual care services utilization for quality assurance and program development.
- Document direct services and ongoing communication with community chaplains and spiritual counselors.
- Attend patient care conferences and interdisciplinary group meetings as a member of the team.
- Provide consultation, education, and support to the interdisciplinary group on spiritual care.
- Recruit community chaplains, spiritual counselors, and spiritual care volunteers to meet patient and family needs by developing community contacts and offering education through congregations about hospice care.
- Provide for funeral or memorial services for patients as requested and plan periodic memorial services for personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
- Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers.
- Actively participate in quality assessment and performance improvement teams and activities.
- Accept and perform other related duties and responsibilities as required.
- Graduate of an accredited seminary or school of theology (BD, MDiv or equivalent); experience working with death and dying, individuals/family/caregiver.
- Ability to work as a member of an interdisciplinary group.
- Comfortable in an interfaith setting.
- Ability to accept different lifestyles, cultures, beliefs, and values.
- Ability to network with community clergy and congregations.
- Knowledge of and commitment to hospice philosophy of care.
- If making home visits, must be a licensed driver with an automobile insured in accordance with state and/or organization requirements and in good working order.
- Hospice experience preferred.
