** Job Summary
** As a
** Physical Therapist (PT)
** with Grand Care, you will provide
** in-home physical therapy services
** according to a physician’s plan of care. You’ll help patients regain mobility and independence after surgery, making a real difference in their recovery.
** Responsibilities
* ** Perform
** home health physical therapy evaluations
** and develop care plans
* Provide therapeutic treatment and instruct patients/caregivers
* Monitor progress and report changes to physicians
* Participate in discharge planning and interdisciplinary case conferences
* Supervise Physical Therapy Assistants per state regulations
** Requirements
* ** Current
** California Physical Therapy License
*** 2+ years of physical therapy experience (
** home health preferred**)
* CPR certification
* Valid driver’s license and insured vehicle
