Registered Nurse – Home Health RN

• Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility determination, admissions processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess needed information from patient’s physical, psychological, social, and spiritual factors that may impact patient and family needs and coordinate intervention by other interdisciplinary team members.
• Initiate communication with attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
• Perform therapeutic, preventative, and rehabilitative nursing procedures as directed by the attending physician.
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely.

• Registered Nurse – Graduate from an accredited RN program, licensed in Tennessee, at least one year of practical nursing experience (preferred ICU, rehab or medical‑surgical).
• Home health experience preferred.
• Valid CPR certification.
• Reliable transportation, current driver’s license and appropriate automobile insurance.
