Nurse Practitioner, In Home Wound Care - Memphis (FT/PT and PRN Roles Available)

Esperta Health is hiring a Wound Care Certified Nurse Practitioner to oversee In-Home Wound Care for our RNs and patients in the Memphis region. The NP will provide and oversee wound care, diagnose and treat wound care patients following evidence-based guidelines, monitor clinical quality of care, and ensure regulatory and company compliance within the assigned region. The role requires an experienced, team-oriented NP who can deliver expert wound care in the home setting using an application-based electronic patient record, in line with state practice guidelines.

Responsibilities

  • Demonstrates proficiency in the full scope of wound care and management, including patient health interviews, review of medical/surgical history, physical assessment, ordering/interpretation of diagnostic testing, diagnosing health problems, managing care, patient education, referrals, and follow-up scheduling; follow Wound Healing Guidelines (WHG) and collaborate with the assigned team; maintain electronic health records
  • Prescribes appropriate treatment orders in accordance with WHG for wound dressings, laboratory testing, cultures, imaging, offloading, and pressure-reducing devices
  • May prescribe non-scheduled legend drugs and manage therapies per state guidelines, in collaboration with or under supervision of a physician as required by state law
  • Maintain professional patient communications during in-home visits and via the application-based electronic patient record
  • Complete documentation meeting regulatory guidelines for clinical care instructions, communications, wound supply orders, and diagnostic results
  • Ensure regional compliance with regulatory and company standards
  • Discharge planning and referrals to case management for post-healing follow-up as indicated
  • Practice safe work practices, infection control, and address safety risks
  • Comply with company policies and procedures
  • Assess for abuse and report as required
  • Provide ongoing patient/caregiver education
  • Perform procedures appropriate to patient condition and provider aptitude as allowed by state boards; procedures include incision and drainage, debridement, hematoma evacuation, suture removal, wound dressings, negative pressure therapy, and skin substitutes
  • Make ethical practice decisions prioritizing safety, effectiveness, and cost
  • Respond to emergencies according to company policies
  • Maintain infection control precautions
  • Be available during scheduled work hours for clinical guidance
  • Collaborate with all company departments
  • Oversee RNs and LPNs in the assigned region and provide orders and guidance as needed
  • Serve as a resource for regional staff and participate in product/treatment evaluations
  • Demonstrate strong customer service; address regional customer concerns with the compliance department
  • Maintain knowledge of HIPAA Privacy, Security, and Breach Notification Rules

Requirements

  • Current, unencumbered nursing license in TN
  • Reliable internet access; company-provided cell phone
  • Willingness to participate in a call schedule
  • Reliable transportation and an unrestricted driver’s license

Qualifications/Education

  • Certified through ANCC or AANP as a Nurse Practitioner
  • WOCN certification or other company-approved wound care certification required
  • U.S. Citizenship
  • Licensed as a professional nurse and advanced practice nurse in good standing
  • Proficient in English
  • Meets physical standards; BLS certification

Experience

  • Minimum of 3 years’ experience treating chronic wounds in hospital, outpatient, or post-acute settings
  • Direct patient care experience
  • Electronic health record proficiency

Skills

  • Strong communication, both oral and written
  • Deep understanding of evidence-based wound care and ability to educate others to improve outcomes
  • Ability to work independently and collaboratively
  • Ability to build professional relationships and mentor colleagues
  • Proficient with Microsoft and Google tools
