Registered Nurse, Wound Ostomy Continence - LTAC

Overview

Responsible for the assessment, planning, implementation, and evaluation of complex wounds, ostomies, and continence care throughout the organization. Provides direct patient care and education to all patients. Establishes standards of care and clinical competencies to meet patients' needs and training programs to ensure clinical expertise. Provides leadership in the development, coordination, and monitoring of wound care program, and system measurements to improve clinical outcomes.

Responsibilities
  • Coordinates and provides wound, ostomy and continence care/services in accordance with established protocols, policies and procedures. Evaluates effectiveness of care/services and makes adjustments/recommendations to improve patient care and services.
  • Plans, organizes and delivers wound management services within the Extended Care Hospital (ECH) consistent with nursing division policies and procedures, demonstrates competencies, and adheres to risk management, infection control, and patient safety practices.
  • Participates as a member of the multi-disciplinary team to develop, implement, and evaluate individual care plans based upon assessment, diagnosis, and expected outcomes of the patient.
  • Assists in developing, implementing and interpreting standards and policies/procedures regarding wound, ostomy and continence care; establishes best practice principles in these areas.
  • Conducts quality assurance programs with the Quality Manager; evaluates effectiveness of care/services and makes adjustments to assure quality and cost-effectiveness of products used for wound, ostomy, and continence care.
  • Assists staff in maintaining competence in skin assessment, wound and ostomy care; provides in-services on skin, wound and ostomy management as needed; demonstrates knowledge, clinical competency, and critical thinking to meet age-specific patient needs.
  • Collaborates with staff to provide education for patients undergoing ostomy or continent diversion surgery; assesses patient/family preoperatively and assists in planning discharge referrals.
  • Provides consultation and guidance to staff in the management of complex wounds and skin integrity; develops comprehensive wound management plans and evaluates patient response to treatment.
  • Serves as a consultant in the management of urinary and fecal incontinence; assesses contributing factors and recommends referrals as indicated; assists in developing continence care plans and education.
  • Identifies learning needs of patients, families, staff and program participants; participates in the development and evaluation of wound care management programs and materials; conducts teaching to achieve desired patient outcomes.
  • Interacts with the health care team, patients, families and visitors in a responsive and courteous manner; serves as a resource to medical, nursing and healthcare teams to achieve expected outcomes.
  • Maintains professional credentials and pursues ongoing education to stay current with clinical concepts and techniques; participates in continuing education and mandatory programs.
  • Provides instruction to nursing and support personnel to complete initial and ongoing assessments and clinical assistance as needed; participates in staff training and quality improvement activities.
  • Assists in orientation and supervision of new staff and students related to patient care and services as part of their training.
  • Maintains a clean, safe environment and adheres to policies, quality improvement, safety, infection control and compliance standards.
  • Prepares and maintains required records and statistics for administrative and regulatory purposes.
Qualifications
  • Bachelor's Degree or equivalent nursing experience (required).
  • Qualified to work with adult, adolescent, neonate and/or geriatric patients as a specialty (required).
  • RN - Registered Nurse;
    State Licensure and/or Compact State Licensure upon hire (required).
  • CWCN - Certified Wound Care Nurse within 1 year (required).
  • Minimum 2 years of experience in wound, ostomy and continence nursing (required).
