How You Make a DifferenceAs a LPN MAT Coordinator under the direction of the MAT Medical Director, you will provide comprehensive, intensive clinical services to members enrolled in the MAT program, both on‑site and off‑site. Your responsibilities include delivering medical and support services such as wellness education, administering injections, coordinating labs and pharmacy services, monitoring vitals, and facilitating communication with psychiatric and medical providers.
Key Responsibilities- Assist in assessing physical, psychological, and social aspects of patients and plan individualized treatment programs in consultation with RNs and other staff.
- Carry out individualized treatment programs, including administering medications, obtaining diagnostic tests, and assisting in medical or minor surgical procedures as directed.
- Adhere to established policies, procedures, and protocols, including counting controlled substances, using proper safety techniques, and responding to health emergencies.
- Document nursing encounters using SOAP charting and communicate essential information to relevant healthcare staff.
- Attend mandatory staff meetings and training sessions to stay updated on healthcare practices and protocols.
Education
- Graduate from an accredited school of nursing.
Experience
- Prefer a minimum of one (1) year clinic experience.
Licenses/Certifications
- Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
- Must be able to obtain and maintain CPR certification.
