Position OverviewWe are seeking an experienced and compassionate Wound Care Nurse to join our team. This role involves assessing and managing acute and chronic wounds, developing tailored treatment plans, and collaborating with a multidisciplinary team to ensure optimal patient care. Responsibilities include conducting evaluations, implementing evidence-based interventions, educating patients and families, and maintaining accurate documentation. - Conduct thorough assessments of patients wounds to determine the appropriate treatment plan.
- Implement and monitor wound care treatments based on evidence-based practices and protocols.
- Educate patients and their families on wound care management, including prevention and home care techniques.
- Collaborate with physicians, nursing staff, and other healthcare professionals to develop and execute individualized care plans.
- Maintain accurate and up-to-date documentation of wound assessments, treatment plans, and progress notes in the patient s medical record.
- Participate in quality improvement initiatives related to wound care and ensure compliance with health regulations.
- Stay informed about the latest advancements in wound care and participate in ongoing professional development.
- Current Licensed Practical Nurse (LPN) license in the state of practice.
- Certification in Wound Care Nursing (CWCN) or equivalent preferred.
- Minimum of 2 years of nursing experience, with at least 1 year in wound care management.
- Strong knowledge of wound healing principles, assessment techniques, and treatment modalities.
