As the Medical Records Assistant at Cordova Wellness & Rehab, you'll be an essential part of our residents' care.

Responsibilities include:

• Maintain, organize, and audit resident medical records.
• Ensure medical records are complete, accurate, and compliant with CMS, state, and facility standards.
• Assist with chart audits and regulatory surveys.
• File, retrieve, and track medical records as requested.
• Monitor physician orders, signatures, and documentation completion.
• Support nursing and administrative staff with documentation needs.
• Assist with admission, discharge, and transfer documentation.
• Perform any other additional tasks as assigned.
• Maintain confidentiality of protected health information, including verbal, written, and electronic communications.

• High school diploma or equivalent required.
• Experience in medical records, healthcare administration, or long‑term care preferred.
• Familiarity with EHR systems and basic computer skills.
