the home first and community therapy services work with individuals experiencing health or social care challenges that place them at risk of hospital admission or reduced independence. Our goal is to provide skilled assessment, rapid intervention, and rehabilitation within the community to prevent unnecessary hospital stays and support people to regain function, confidence, and autonomy.

as a physiotherapist in this service, you will:

  • support individuals who are experiencing an acute deterioration or crisis, enabling them to stay at home safely rather than requiring hospital admission.
  • contribute to early discharge pathways, helping people transition from hospital settings back to the home environment as quickly and safely as possible.
  • provide targeted rehabilitation and reablement interventions to support recovery of mobility, functional ability, and overall health and wellbeing.
  • work collaboratively with multidisciplinary colleagues including occupational therapists, support workers, community nurses, and adult social care teams to deliver holistic, person‑centred care.

main duties of the job

we are looking for band 6 physiotherapists to provide an effective and efficient therapy service. This post requires a level of personal responsibility for someone highly motivated, resilient and proactive with the ability to prioritise and manage their own allocated caseload, use their initiative and the ability to work autonomously and as part of a team.

demonstrate excellent communication, organisational skills and be able to adapt to a changing and pressurised environment. Working with occupational therapists and technical instructors to carry out comprehensive functional assessments and interventions to help patients live as independently as possible. The therapy teams operate in an integrated way whereby therapists are encouraged to work synergistically in order to provide the best possible outcomes for patients.
