Overview:
We are seeking a compassionate and dedicated Licensed Mental Health Counselor (LMHC) to provide psychotherapy and behavioral health support to residents in a long-term care and skilled nursing facility setting.
The ideal candidate will have experience working with geriatric populations, addressing emotional, behavioral, and adjustment issues common among individuals coping with chronic illness, loss of independence, and life transitions.
This role focuses on improving quality of life, emotional well-being, and adherence to medical and rehabilitation goals through evidence-based psychotherapy.
Key Duties and Responsibilities- Conduct comprehensive mental health assessments to determine residents’ emotional and psychological needs.
- Develop and implement individualized treatment plans based on assessment findings and resident goals.
- Provide psychotherapy for a range of conditions including depression, anxiety, grief, trauma, behavioral changes, and adjustment disorders.
- Support residents coping with chronic illness, pain, and cognitive changes
, using therapeutic interventions tailored to functional abilities. - Address behavioral and emotional disturbances such as agitation, aggression, withdrawal, or inappropriate behaviors.
- Collaborate closely with the interdisciplinary care team—including physicians, nurses, rehabilitation therapists, and social workers—to coordinate holistic care.
- Maintain accurate clinical documentation and progress notes in compliance with facility and state regulations.
- Provide family education and support when appropriate, promoting understanding and engagement in the resident’s care plan.
- Participate in ongoing monitoring and evaluation of treatment effectiveness, adjusting care as needed.
- Active state licensure as an LMHC in Massachusetts.
- Experience in geriatric, rehabilitation, or long-term care settings strongly preferred.
- Solid knowledge of depression, anxiety, grief, adjustment disorders, cognitive changes, and behavioral health management in older adults.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Strong communication, documentation, and interpersonal skills.
- Compassionate, patient-centered approach with sensitivity to the emotional and cognitive needs of older adults.
Job Type: Full-time
Pay: $35.00 - $40.00 per hour
