• Willing to travel to different areas of the surrounding suburbs

• Registered nurse conducts in-home patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed by physicians
• Registered nurse monitors patient vital signs and condition, and report any changes to the healthcare team
• Registered nurse educates patients and their families on proper care techniques and disease management
• Collaborate with other healthcare professionals to ensure comprehensive patient care
• Document all patient interactions, observations, and care provided accurately

• Valid Registered Nurse (RN) license in the state of practice, Driver's License, & proof of insurance.
• Minimum of one year of experience in direct patient care, preferably in home health or hospital setting.
• Proficiency in medical terminology and basic life support (BLS) certification

• Familiar with OASIS documentation.
• Knowledge of wound care management and infection control practices
