POSITION SUMMARY

The Home Health Licensed Practical Nurse (LPN) performs treatment, education, and provides appropriate age‑specific services under the direction of a Registered Nurse (RN).

PRIMARY

ACCOUNTABILITIES
  • Provides nursing care and prescribed treatments for patients in their homes according to the physician’s plan of care, under the supervision of an RN.
  • Reports any necessary alterations to the plan of care, based on observed changes in the patient's condition, to the supervising RN for appropriate action.
  • Verifies the safety of all treatment procedures, patients, and the home environment at all times.
  • Adheres to all Health First, Inc., and Health First Home Care policies and procedures to maintain compliance and uphold care standards, complies with State and Federal regulations and The Joint Commission (TJC) standards for Home Care service delivery, ensuring regulatory and quality standards are met.
  • Participates actively in Quality Improvement initiatives to enhance patient care and service delivery.
  • Assists in organizing patient care, including scheduling and documentation reviews, as requested by the supervisor.
  • Communicates adverse findings in patient conditions to the Case Manager or PI Supervisor and document all findings accurately in the medical record.
  • Notifies the PI Supervisor of any changes in projected visit schedules or hours worked to ensure compliance with weekly work limits.
  • Administers medication and intravenous (IV) therapy, participating fully in Telehealth initiatives, and to meet or exceed annual Press Ganey Patient Satisfaction goals.
MINIMUM QUALIFICATIONS
  • Education:

    Technical Diploma in Practical Nursing.
  • Work Experience:

    None
  • Licensure:
    Licensed Practical Nurse (LPN) licensure in the State of Florida.
  • Drivers License and clean Motor Vehicle Report (MVR) maintained.
  • Certification:
    American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained.
  • Background Check:
    Satisfies the State of Florida, Agency for Healthcare Administration (AHCA) Level II Background Screening.
  • Skills/Knowledge/Abilities:
    • Ability to accept assignments willingly, regardless of geographic territory, and attends at least 80% of scheduled staff meetings to stay informed and engaged.
    • Good communication skills.
    • Proficiency in MS Office, electronic records and other systems and software relevant to role.
    • Ability to interact with other professionals, families, and patients.
    • Ability to work in stressful environments and/or situations.
    • Ability to decide concern appropriate patient care.
    • Ability to respond appropriately to life threatening situations.
    • Ability to assess, interpret data and educate patients and families of various ages and levels of understanding.
    • Ability to work independently within time constraints and concentrate under multiple demands.
PREFERRED QUALIFICATIONS
  • Work Experience:

    One (1) year experience in Home Care/Health.
PHYSICAL REQUIREMENTS
  • Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, or crouching frequently.
  • May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.
  • May require moving in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders, or overhead.
  • May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
  • May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
  • May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
  • Communicating with others to exchange information.
  • Visual acuity and hand‑eye coordination to perform tasks.
  • Workspace may vary from open to confined.
  • May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
