Overview

POSITION SUMMARY The Social Worker LCSW is fully engaged in providing quality/no harm, customer service and stewardship by utilizing advanced psychosocial skills to facilitate the coordination of patient care through assessment, planning, intervention and evaluation. The Social Worker LCSW uses current knowledge to effectively evaluate, promote and utilize hospital and community resources and services to achieve optimal outcomes, serving as a role model for coordination of psychosocial services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers.

Primary

Accountabilities
  • Collaboratively determines desired discharge goals and communicates all discharge plans and options to patient/family, physicians and staff members.
  • Provides counseling and researches placement alternatives for substance abuse, mental health, homeless and indigent patients.
  • Facilitates and participates in transfer of Baker Act and Marchman placements to mental health facilities. Provides therapeutic interventions with patients and families as needed.
  • Completes mandatory reporting processes.
  • Facilitates referrals for post-discharge psychosocial and therapeutic needs.
  • In a crisis situation where safety of the patient or others is in immediate jeopardy, the LCSW can Baker Act a patient.
  • Mentors social work students from local universities during their internship in the department.
  • Provides clinical supervision for social workers in the department and those pursuing their LCSW.
  • Identifies system/process breakdowns and improvement opportunities, documenting for the Performance Improvement Plan.
  • Intervenes and recommends appropriate referrals to resolve system barriers to patient progression, collaborating with other disciplines as directed.
  • Communicates avoidable delays to the RN Case Manager for collaboration and documentation in MIDAS.
  • Ensures compliance with Federal and state regulatory standards of care, identifying risk management and quality issues, and intervening or referring as indicated.
MINIMUM QUALIFICATIONS
  • Education:

    Master’s of Social Work
  • Licensure:
    Current Florida LCSW license
  • Certification:
    Current American Heart Association Basic Life Support Healthcare Provider Completion Card upon hire and maintained
  • Work Experience:

    One (1) year of previous social work experience
  • Knowledge/Skills/Abilities:
    • Basic computer skills including knowledge of MS Office – Outlook and Word
    • Current knowledge of healthcare economic issues impacting acute care
    • Working knowledge of social work scope of practice and current issues impacting healthcare
    • Understanding of payer/reimbursement practices and regulations
    • Ability to understand and comply with Federal and state regulatory standards of care
    • Ability to effectively handle multiple demands in a short timeframe
Preferred Qualifications
  • Certification: CCM or ACM Case Management Certification
  • Work Experience:

    Case Management experience in Hospital setting
PHYSICAL REQUIREMENTS (Sedentary)
  • Majority of time involves sitting or standing; occasional walking, bending, stooping
  • Long periods of computer time or at workstation
  • Light work that may include lifting or moving objects up to 20 pounds with or without assistance
  • May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise
  • Communicating with others to exchange information
  • Visual acuity and hand-eye coordination to perform tasks
  • Workspace may vary from open to confined
  • May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle
