ResponsibilitiesResponsible for the use of advanced nursing processes to identify high‑risk, acutely ill patients, and patients with chronic disease. The role improves health outcomes through a population‑health platform and is accountable for assessing, planning, educating, monitoring, evaluating, and coordinating services for patient populations and their families, fostering quality, continuity, and appropriate utilization of health‑care resources throughout the continuum of care. Facilitates coordinated utilization of resources to maximize health outcomes, patient/family satisfaction, and financial outcomes.
Assists patients, families, and caregivers in securing necessary medications, equipment, and community resources to facilitate wellness. As part of a multidisciplinary team, consults with other health‑care team members to coordinate preventive care and chronic disease management.

Education:
Bachelor of Science in Nursing (BSN). Associates considered if BSN obtained within 3 years. - Graduated from a CCNE or ACEN accredited nursing program.
- Current RN license in Ohio.
- Three (3) years clinical experience in care coordination.
- Strong experience with diabetic populations and other chronic diseases.
- Strong knowledge in promoting healthy lifestyles and preventive screenings.
- One (1) year of Care Management experience preferred.
- Knowledge of Epic EMR software preferred.
- Other related experience for specialty care management.
- Demonstrated ability to manage time, promote departmental and professional growth, use electronic software applications related to care management, act as subject‑matter expert for clinical staff education, implement care management activities, attend required educational offerings, participate in continual improvement of care management processes, understand population health practices, quality improvement tools, and use MS Office.
- Excellent customer service, written and verbal communication skills, ability to manage multiple tasks, and ability to set goals independently and prioritize work.

Sedentary; occasionally exert up to ten pounds of force.

Compensation$37.40/hr - $56.11/hr the range represents base salary exclusive of bonuses or differentials. Factors such as experience and location are considered when determining the starting rate.
